The aim of the project is to deinstitutionalize the services provided, stimulate people with disabilities to take up activities, counteract discrimination on the basis of disability and social exclusion of people with disabilities, enable OZN to participate in the life of the local community and enable them to realize their right to an independent life thanks to the support of assistants.

The project is addressed to 30 adult OZN from the province. Lesser Poland, regardless of their status on the labor market (unemployed, economically inactive, employed).

According to the terms of the competition, the services of a personal assistant can only be used by disabled people who have long-term physical, mental, intellectual or sensory impairments which, in interaction with various barriers, may significantly impede their participation in social life, on the basis of equality. with other people. People with disabilities other than those indicated above cannot be qualified to participate in the project regarding the provision of assistant services.

Pursuant to the Regulations, persons with a certificate of mild disability cannot constitute more than 10% of the total final beneficiaries of the project; when verifying this indicator, final beneficiaries with a mild degree of disability due to intellectual disability, pervasive developmental disorders, long-term, chronic mental disability, multiple disabilities, or breast cancer are not taken into account.

The final beneficiaries of the project in which the assistant service is provided may be participants of occupational therapy workshops, community self-help homes or other facilities - provided that the scope of support implemented in the project does not overlap with the activities undertaken in the facility for a given participant, in accordance with the individual rehabilitation program and therapy or an individual supportive and activating treatment plan or another support plan/program developed in accordance with the requirements resulting from the regulations governing the operation of the facility.

As part of the project, we offer the following support free of charge:

1. Comprehensive diagnosis of the life situation along with preparation of IPD

2. Care manager services

3. Personal assistance services (average 171 hours of services/Participant)

4. Assessment of the increase in independence. 


The project candidate must present:
1. copies of the disability certificate or equivalent document;
2. a declaration that he does not participate in other projects financed from PFRON funds aimed at professional activation of people with disabilities.
3. a statement confirming the place of residence in the area covered by the project implementation
4. a set of completely and correctly completed recruitment documents - recruitment form, declaration of participation in the project and consent to data processing and the GDPR clause.
The final beneficiary with a temporary disability certificate, the validity of which expires during participation in the project, may benefit from support before the next disability decision is issued - provided that the costs of the beneficiary's participation in the project may be considered eligible only when another disability decision is issued. degree of disability or a disability certificate (covering the period of receiving support in the project).
The order of applications will determine qualification for the project.

Recruitment is conducted continuously until places are filled. People who were not qualified to participate in the Project due to lack of places will be included on the reserve list.
