My personal assistant III_MAZOWSZE

We are starting the implementation of the project entitled "My personal assistant III_MAZOWSZE" co-financed by the State Fund for Rehabilitation of Disabled Persons (PFRON) as part of the "Strength of Possibilities" competition (1/2025).

The project is aimed at 60 adults with severe or moderate disabilities living in the Masovian Voivodeship, regardless of their employment status. The main goal is to increase participants' independence and active participation in social life through individualized support from assistants and the activities of a care manager.

According to the competition terms, personal assistant services are available only to individuals with disabilities who have long-term physical, mental, intellectual, or sensory impairments, which, in conjunction with various barriers, may significantly hinder their ability to participate in social life on an equal basis with others. Individuals with disabilities other than those listed above cannot be considered for participation in the personal assistant services project.

The ultimate beneficiaries of the project in which the assistance service is provided may be participants of occupational therapy workshops, community self-help homes or other facilities – provided that the scope of support implemented in the project does not duplicate the activities undertaken in the facility for a given participant, in accordance with the individual rehabilitation and therapy program or individual plan of supportive and activation procedures or other support plan/program developed in accordance with the requirements arising from the regulations regulating the operation of the facility.

A person with a disability cannot use personal assistance services in a project financed as part of commissioned tasks if, at the same time, he or she uses these services in a project financed from other public funds.

As part of the project, we offer the following support free of charge:

1. Comprehensive diagnosis of the life situation along with the preparation of IPD

2. Care manager services

3. Personal assistance services (average 200 hours of services/BO)

4. Assessment of the increase in the level of independence.

The project candidate must present:

1. A complete set of correctly completed recruitment documents, i.e.

– a recruitment form containing a declaration of non-participation in other projects financed by PFRON and a declaration of residence in the area covered by the project,

– declaration of participation in the project and consent to data processing

– GDPR clause.

2. Copy of the disability certificate or equivalent document.

Recruitment is conducted continuously until places are filled. People who were not qualified to participate in the Project due to lack of places will be included on the reserve list.
