We are starting the implementation of the project entitled "Your Assistant IV_Ma\u0142opolska" co-financed by the State Fund for Rehabilitation of Disabled Persons (PFRON) as part of the "Strength of Possibilities" competition (1\/2025).<\/p>\n\n\n\n

The project is aimed at 60 adults with a severe or moderate degree of disability, residing in the Ma\u0142opolska Voivodeship (Tarn\u00f3w, Tarn\u00f3w subregion, Bochnia County), regardless of their labor market status. The main goal is to increase participants' independence and active participation in social life through individual assistant support and the activities of a care manager.<\/p>\n\n\n\n

According to the terms of the competition, personal assistant services may only be used by people with disabilities who have long-term physical, mental, intellectual or sensory impairments, which, in interaction with various barriers, may significantly hinder their participation in social life on an equal basis with others. Persons with disabilities other than those indicated above cannot be qualified to participate in a project involving the provision of assistance services.<\/u><\/strong><\/p>\n\n\n\n


The ultimate beneficiaries of the project in which the assistance service is provided may be participants of occupational therapy workshops, community self-help homes or other facilities \u2013 provided that the scope of support implemented in the project does not duplicate the activities undertaken in the facility for a given participant, in accordance with the individual rehabilitation and therapy program or individual plan of supportive and activation procedures or other support plan\/program developed in accordance with the requirements arising from the regulations regulating the operation of the facility.<\/p>\n\n\n\n

A person with a disability cannot use personal assistance services in a project financed as part of commissioned tasks if, at the same time, he or she uses these services in a project financed from other public funds.<\/strong><\/p>\n\n\n\n

As part of the project, we offer the following support free of charge:<\/h2>\n\n\n\n

1. Comprehensive diagnosis of the life situation along with preparation of IPD<\/p>\n\n\n\n

2. Care manager services<\/p>\n\n\n\n

3. Personal assistance services (average 200 hours of services\/BO)<\/p>\n\n\n\n

4. Assessment of the increase in the level of independence. <\/p>\n\n\n\n


The project candidate must present:<\/h3>\n\n\n\n

1. A complete set of correctly completed recruitment documents, i.e.<\/p>\n\n\n\n

\u2013 a recruitment form containing a declaration of non-participation in other projects financed by PFRON and a declaration of residence in the area covered by the project,<\/p>\n\n\n\n

\u2013 declaration of participation in the project and consent to data processing <\/p>\n\n\n\n

\u2013 GDPR clause<\/p>\n\n\n\n

2. Copy of the disability certificate or equivalent document.<\/p>\n\n\n\n

Recruitment is conducted from April 20, 2026 on a continuous basis until all positions are filled. <\/strong><\/p>\n\n\n\n

Applications will be accepted in person at the project office, via traditional mail or e-mail.<\/p>\n\n\n\n

Qualification for the project will be determined by the order in which applications are received. Those who are not accepted due to a lack of available spots will be placed on a reserve list.<\/p>\n\n\n\n
