In practice, it becomes an experience of the entire system of relationships—partnerships, family, friendships, and the workplace. A cancer diagnosis introduces changes into these relationships, and very often alters their dynamics.

One of the more difficult questions that sometimes arise is: Why did some stay and others disappear? The answer is rarely simple, because people's reactions to the illness of a loved one are complex and influenced by many factors.

Illness as a borderline experience

A cancer diagnosis confronts issues that are often pushed aside in everyday life: helplessness, fear of death, the fragility of the body and strength, lack of control, and mortality. For some people, closeness in such a situation deepens the bond – there emerges greater authenticity, tenderness, mindfulness, and a willingness to be present "here and now.".

For others, this experience can be too difficult and overwhelming. It can trigger fear, helplessness, and sometimes unprocessed personal losses. Contact with a loved one's illness can reflect one's own fears and limitations. Not everyone has the mental resources to function under such high stress for a long time.

Maintaining a relationship during cancer treatment also requires certain resources and competencies: the ability to regulate one's own emotions, tolerate uncertainty, and endure situations without simple solutions. In a culture that promotes agency and quick "fixes," being present alongside someone's helplessness can be particularly challenging. Silence, avoidance, excessive optimism, or withdrawal don't always reflect a lack of concern. Sometimes they're strategies for coping with anxiety, sometimes helplessness. This doesn't mean they don't hurt—but their source may be more related to the resources and willingness of the support person than to the person with the illness.

What was the relationship built on?

The previous structure of the relationship, its foundations, also matters. If the relationship was based primarily on shared activities and defined roles, illness can shake its foundations. Energy levels, availability, and sometimes identity change. This requires a renegotiation of the rules of intimacy. Relationships based on emotional openness and flexibility are more likely to adapt to change, but even they experience crises. Illness also places a strain on those supporting them—physical and psychological. It's worth discussing this, not to relativize the suffering of the sick person, but to realistically understand the entire system.

Relational loss as a real experience

The separation of loved ones can be an additional, painful experience. It's a form of loss—sometimes sudden, sometimes gradual. You can understand the psychological mechanisms and simultaneously feel grief, anger, and disappointment. These emotions are appropriate, and you have a right to them.

In psycho-oncology, we increasingly speak of the "secondary losses" that accompany illness—the loss of control, plans, social roles, and even some relationships. Recognizing these losses is an important element of the adaptation process.

What might emerge from this experience?

Cancer isn't a moral test of love or a test of character. More often, it's a moment when boundaries—our own and those of others—are revealed. This is painful, but also ordering. Although we have no control over who stays, we can gradually regain control over the kinds of relationships we need and are ready for. For some people, the experience of illness becomes an impulse to build more authentic bonds or to redefine their own needs. Sometimes the circle of support shrinks, but it becomes more qualitative. And relationships that withstand confrontation with fragility often take on new depth. And this isn't about the illness "being there for a reason." Rather, it means that even in its shadow, it's possible to build closeness full of sincere commitment, presence, and love.

***

Author: Katarzyna Binkiewicz, M.A.

Choroba nowotworowa rzadko dotyczy wyłącznie ciała.
